ABSTRACT

BACKGROUND@#This study aimed to evaluate the correlation between long non-coding RNA (lncRNA)-related single nucleotide polymorphisms (SNPs) and susceptibility to silicosis.@*METHODS@#First, RNA-sequencing (RNA-seq) data were comprehensively analyzed in the peripheral blood lymphocytes of eight participants (four silicosis cases and four healthy controls) exposed to silica dust to identify differentially expressed lncRNAs (DE-lncRNAs). The functional SNPs in the identified DE-lncRNAs were then identified using several databases. Finally, the association between functional SNPs and susceptibility to silicosis was evaluated by a two-stage case-control study. The SNPs of 155 silicosis cases and 141 healthy silica-exposed controls were screened by genome-wide association study (GWAS), and the candidate SNPs of 194 silicosis cases and 235 healthy silica-exposed controls were validated by genotyping using the improved Mutiligase Detection Reaction (iMLDR) system.@*RESULTS@#A total of 76 DE-lncRNAs were identified by RNA-seq data analysis (cut-offs: fold change > 2 or fold change < 0.5, P < 0.05), while 127 functional SNPs among those 76 DE-lncRNAs were identified through multiple public databases. Furthermore, five SNPs were found to be significantly correlated with the risk of silicosis by GWAS screening (P < 0.05), while the results of GWAS and iMLDR validation indicated that the variant A allele of rs1814521 was associated with a reduced risk of silicosis (OR = 0.76, 95% CI = 0.62-0.94, P = 0.011).@*CONCLUSION@#The presence of the SNP rs1814521 in the lncRNA ADGRG3 is associated with susceptibility to silicosis. Moreover, ADGRG3 was found to be lowly expressed in silicosis cases. The underlying biological mechanisms by which lncRNA ADGRG3 and rs1814521 regulate the development of silicosis need further study.

ABSTRACT

Objective:To develop a grading prediction model of traumatic hemorrhage volume based on deep learning and assist in predicting traumatic hemorrhage volume.Methods:A retrospective observational study was conducted based on the experimental data of pig gunshot wounds in the time-effect assessment database for experiments on war-traumatized animals constructed by the General Hospital of the Chinese People's Liberation Army. The hemorrhage volume data of the study population were extracted, and the animals were divided into 0-300 mL, 301-600 mL, and > 600 mL groups according to the hemorrhage volume. Using vital signs indexes as the predictive variables and hemorrhage volume grading as the outcome variable, trauma hemorrhage volume grading prediction models were developed based on four traditional machine learning and ten deep learning methods. Using laboratory test indexes as predictive variables and hemorrhage volume grading as outcome variables, trauma hemorrhage volume grading prediction models were developed based on the above fourteen methods. The effect of the two groups of models was evaluated by accuracy and area under the receiver operator characteristic curve (AUC), and the optimal models in the two groups were mixed to obtain hybrid model 1. Feature selection was conducted according to the genetic algorithm, and hybrid model 2 was constructed according to the best feature combination. Finally, hybrid model 2 was deployed in the animal experiment database system.Results:Ninety-six traumatic animals in the database were enrolled, including 27 pigs in the 0-300 mL group, 40 in the 301-600 mL group, and 29 in the > 600 mL group. Among the fourteen models based on vital signs indexes, fully convolutional network (FCN) model was the best [accuracy: 60.0%, AUC and 95% confidence interval (95% CI) was 0.699 (0.671-0.727)]. Among the fourteen models based on laboratory test indexes, recurrent neural network (RNN) model was the best [accuracy: 68.9%, AUC (95% CI) was 0.845 (0.829-0.860)]. After mixing the FCN and RNN models, the hybrid model 1, namely RNN-FCN model was obtained, and the performance of the model was improved [accuracy: 74.2%, AUC (95% CI) was 0.847 (0.833-0.862)]. Feature selection was carried out by genetic algorithm, and the hybrid model 2, namely RNN-FCN* model, was constructed according to the selected feature combination, which further improved the model performance [accuracy: 80.5%, AUC (95% CI) was 0.880 (0.868-0.893)]. The hybrid model 2 contained ten indexes, including mean arterial pressure (MAP), hematocrit (HCT), platelet count (PLT), lactic acid, arterial partial pressure of carbon dioxide (PaCO 2), Total CO 2, blood sodium, anion gap (AG), fibrinogen (FIB), international normalized ratio (INR). Finally, the RNN-FCN* model was deployed in the database system, which realized automatic, continuous, efficient, intelligent, and grading prediction of hemorrhage volume in traumatic animals. Conclusion:Based on deep learning, a grading prediction model of traumatic hemorrhage volume was developed and deployed in the information system to realize the intelligent grading prediction of traumatic animal hemorrhage volume.

ABSTRACT

Objective To investigate the clinical effect of methotrexate combined with leflunomide in the treatment of patients with rheumatoid arthritis.Methods 660 patients with rheumatoid arthritis were selected,they were divided into research group and the control group according to the random number method,with 330 cases in each group.The control group was treated with methotrexate alone,while the research group was treated with leflunomide and methotrexate.The clinical effect and adverse reaction of the two groups were compared after 6 months.Results Compared with before treatment,the pain index[(1 .43 ±0.32)vs.(1 .83 ±0.52)],time of morning stiffness [(0.77 ±0.5)h vs.(1 .1 8 ±0.44)h],grip strength[(1 58.65 ±53.45)mmHg vs.(1 33.29 ±44.1 2)mmHg], swelling[(6.81 ±2.32)vs.(9.32 ±2.43)]and the swelling index[(1 .54 ±0.26)vs.(1 .70 ±0.33)]in the two groups after treatment were improved significantly,and the differences were statistically significant(t =3.84,5.24, 3.54,4.05,6.95,4.33,5.22,3.69,2.98,5.64,all P 0.05).Conclusion Methotrexate combined with leflunomide has good effect in the treatment of patients with rheumatoid arthritis,which can significantly improve clinical symptoms,as well as effectively reduce the related indicators of rheumatoid arthritis in active phase,with good safety,it is worth promoting.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate whether myocardial bridging (MB) is an independent risk factor for coronary atherosclerosis proximal to the bridge site in the left anterior descending coronary artery (LAD) in diabetic patients.</p><p><b>METHODS</b>From March 2011 to December 2012, 9862 patients with suspected coronary disease underwent coronary computed tomography angiography (CCTA) using a dual-source CT scanner. The baseline clinical characteristics (age, gender, smoking history, presence of hypertension, dyslipidemia, diabetes mellitus, family history of heart attack and body mass index) and the results of CCTA were reviewed. Two radiologists evaluated the MB and coronary atherosclerosis stenosis (CAS) over 50% in the LAD and made a diagnosis by consensus. Significant independent risk factors for CAS were investigated by logistic regression analysis.</p><p><b>RESULTS</b>Of the 2345 patients identified to have diabetes mellitus, 1373 had MB, among whom 827 had coronary atherosclerosis proximal to the bridge site; 972 of the diabetic patients were free of MB, among whom 254 had coronary atherosclerosis at the equivalent site. None of the patients had CAS in the tunneled segment. After adjusted for clinical data, logistic regression analysis showed that MB in the LAD was significantly correlated with coronary atherosclerosis in the proximal LAD in diabetic patient (OR=3.91) and non-diabetic patients (OR=2.69) (P<0.05).</p><p><b>CONCLUSION</b>In diabetic patients, atherosclerosis occurred frequently in the segment proximal to MB in the LAD, and MB in the mid-LAD is an independent risk factor for CAS in the proximal LAD.</p>

Objective To study prospectively HRCT-pathological correlatiion of the small solitary pulmonary nodules in 52 cases.Methods The characterstics of each nodule's margin,internal and peripheral on high resolution CT images was observed with double blind method.Postoperative specimens were fixed in the Heitzman standen method,then specimes were scanned by computed tomography,HRCT signs of nodule were also viewed at same way.Results The results showed :the diameter and distribution of benign and malignant pulmonary nodules was not obviously;the density of benign nodules was more homogeneous than that of malignant nodules.The margin of malignant had slight lobulation in 51%;Halo sign and the peripheral pulmonary emphysema was showed in benign nodules. ABSTRACT

Objective To evaluate CT scanning value in the diagnosis of pulmonary lymphangioleiomyomatosis(PLAM).Methods CTfindings of PLAM proved pathologically in 13 cases were retrospectively analyzed including distribution,morphosis and complication oflesions.Results 3 were lobar or segmental lesion and 10 were desseminated lesion.The fine nodules and pulmonary cystic space were showed on conventional CT scanner.The fine nodules(1~2 mm in diameter)and pulmonary cyst spaces(1 ~4 mm walled )were seen on high resolution CT.The most of cystic spaces were round.13 cases of pathologic species were retrospectively reviewed.The results showed that 12 cases were PLAM and 1 case was pulmonary tuberous sclerosis.Conclusion In young female, the fine nodules or pulmonary cyst spaces were presented on CT scan,PLAM should been strongly suspected.
